At the end of each test you must save a copy of the
Results page before submitting the test.

If you do not receive a copy of the test by email after you have
submitted it, then you will need to contact us to see if we
received our copy.  If we did not, then we will contact you
and ask you to send a copy of the Results page to the
college as verification for your academic file that
you did take the test and the score earned on it.


CLOSED BOOK TEST CONTRACT
Submit this form ONLY if you are requesting the password for a "closed book" test
or if you are requesting a password for a restricted lesson.

You must submit this agreement before taking the Closed Book Test in order for you to receive the Password for the test and for your grade to be credited to you.

If the test is not "closed book" but is, instead, an "open book" test, then do not submit this form. If the test is "open book" then submit an "open book" Password Retrieval form.  (Click HERE to access that form.)

By submitting this form you agree to a legal contract with Salt Lake Bible College and Salt Lake Baptist College.

NOTE:  We have found it necessary to remind all students that they are required to observe proper letter-case when submitting information or sending correspondence to us.  Use of all upper-case letters or all lower-case letters will result in this form being rejected unless such use is specifically required.  Use of proper letter-case is also required for any other correspondence sent to the college.  We have observed that there are many problems in this area of use of improper letter-case; but, the lack of proper capitalization of the first letter of proper names seems to be the most prevelant problem.  Therefore, it is recommended that students exercise extra caution when inputting their first name and last name on this form, and all other forms or other correspondence sent to the college, to make sure that the first letter of each of your names is capitalized.
Example: If your name is John Smith, then make sure that you submit John Smith and not john smith or JOHN SMITH.

By submitting this form you signify that you understand and that you agree that should you not fulfill the terms of this contract, such failure to be known by the term "cheating," that you will be expelled from Salt Lake Bible College and/or Salt Lake Baptist College and that your school record will have the notation added, "Expelled for cheating" and that all credits previously earned will be declared null and void.

If you do not have a calendar handy to figure the number of weeks of your attendance, figured from
the date of your Begin Sign In form to the date of your Finish Sign In form or, if you are not yet
finished with the course, today's date, then you can open a Date Calulator on this site,
CLICK HERE, and follow the instructions to calculate your attendance.
Make sure you check the box entitled, "Include end date in calculation (1 day is added)".
When the time span has been calculated for you, look for "Alternative time units" and
under that heading you will find the number of weeks and it will be marked
"(rounded down)" which is what you need to enter for the number of weeks
of your attendance in the course. Make a note of the number of weeks and
then close that window and enter the number of weeks into the
"Weeks of your Attendance in this Course" field which
is situated immediately below these instructions.

General information for all students requesting passwords:

We have found it necessary to remind students that it is their responsibility to fulfill all of the attendance/study/testing requirements before taking the Lesson Tests or the Final Tests. If the specified requirements have not been met before taking any particular test, then the test will be disallowed and the student will have to retake the test when the requirements have been met.

PASSWORD: When this form is received by the college, the password for the closed-book test will then be sent to you within 1 - 2 days; not including weekends when the college offices are closed.
