You only need to submit this form if you are dormant in your studies for more than 30 days.

   It is required that all students maintain active contact with the college.

   Our policy is to place the student on "inactive" status after 60 days with no contact and "dropped" status after 90 days.

   If you are turning in any tests, forms, etc., or sending in email questions or other email correspondence, then you are already maintaining active contact.  If you are, then you do not need to file this form.

For clarification: You only need to submit this form if there is no activity of any kind in your file for more than 30 days.  As long as you are submitting any tests, forms, or other communication to us then your file registers activity and an Active Status form is not necessary.

   If you are currently in the process of writing your thesis or are taking courses that do not require turning in section tests or password forms on a regular basis, such as OT Survey and NT Survey and other long term courses, and you are not turning in tests or forms for other courses then please contact us once a month via this form verifying that you are still an active student.
