Transcript Submission

 

This form is to be used only by Extension Campuses of SLBC.
It is not for individual use.

If you are an individual and want to send your transcripts
to us, you may scan them and attach them to an email or
you may have the previous school send them to us direct.

 

When you submit the transcript, you will receive a confirmation listing all of the information submitted.

If any of the information is wrong, click the back arrow to return to the form and correct any mistakes before submitting form again.
CAUTION: Do not click "return to form" or all of the information on the form will be erased and you will have to enter it all again instead of just correcting the wrong information.

Use a separate form for each student transcript submitted.

INSTRUCTIONS for TRANSCRIPT DATA ENTRY:
These instructions are for those Extension Campuses that follow our USA system.

Extensions in other countries:
- Extensions in other countries do NOT need to follow this example of 4.0 as the highest grade.
- Instead, they may use the system appropriate for the country in which they operate.
- For convenience sake and to make the transcript useable both in the USA and in the country
   of their operation, non-USA Extensions can record only the percentage grade earned for the course rather than
   the equivalent under the "4.0" system.
COURSE = the name of the course.
CREDITS = the number of credits earned for the course.
GRADE = the grade for the course in 0-4 format.
     To get the 0-4 format multiply the percentage by 4 and
   enter the result as the grade.
    Process:  a grade of 90% is .90, to get the 0-4 format, multiply by 4.
         Example 1 is 90%:  .90 X 4 = 3.6 as the grade.
         Example 2 is 75%:  .75 X 4 = 3.0 as the grade.
         Example 3 is 69%:  .69 X 4 = 2.76 as the grade.
NOTE 1:  Anything below 69%, which translates to
	  a 2.76 in the "0-4" format, is a failing grade for the course!
