As an affiliate campus of Salt Lake Bible College (hereafter called SLBC) we, the Affiliate Campus, hereby agree to abide by all requirements and operating procedures as listed in the standard Affiliate Campus Agreement and to meet all financial obligations to SLBC.

Signature of Chancellor or other authorized representative of the school signifies agreement to the terms of these requirements as binding for initial certification and for continued certification of the school as an affiliate campus of Salt Lake Bible College.

Submission of this form is for tentative status.  You must also mail us the hard copy so that we can have it on file in order for your status to be changed from "tentative" to "approved."

Submission of this online form indicates that I have read and that
I understand and agree to all requirements for certification as an
affiliate of Salt Lake Baptist College as outlined in the
Affiliate Campus Agreement
I also understand that this online form only gives me tentative
status and must be followed up with the proper form submitted
by regular mail.  I also understand that I will receive approved
status upon receipt of that form by SLBC and pursuant to their
subsequent approval of my application.

 

REGISTER FOR "APPROVED" STATUS:
 If you have not already done so, go to the Affiliate Campus Agreement
and print it out and fill out and return the indicated page to us.  When we
receive that page in the regular mail, it will be placed in your file and, upon
our approval of your application, your status will be changed from
"tentative" to "approved."  Your "Approved" status will continue until the
end of the school year for which you applied on the condition that you
continue to honor the terms of the Affiliate Campus Agreement
When we receive your application by regular mail for your file, you will
receive your Certificate of Approved Status as an Affiliate Campus of
Salt Lake Bible College.
